HOOAH 4 YOUR Personal Health

HOOAH 4 HEALTH aids in the assessment and planning of personal health initiatives.

The following interactive Health Goals Checklist will aid you in determining your personal health goals. For instance, if you feel that you need to focus on learning to relax, then check one of the buttons in the high range. If you feel that this is not a goal for your personal health, then check one of the buttons in the low range. Once you fill out the form and submit it, you will be shown a report of the compiled results. The results will be sent to the HOOAH 4 HEALTH program coordinator and will be used as a tool to help us put the program together.
