GOALS /OBJECTIVES for TARGETING HEALTH Program at [Sponsoring Agency]

TARGETING HEALTH (TH)

  1. Fifty percent of participants will take the opportunity to become more aware of strategies to improve their health.

    1. Market a minimum of one activity per month via cc mail, flyers, and posters to all participants.
    2. Monitor change in awareness of participants by pre and post-tests of an activity.
    3. Provide quarterly home mailings in the form of a newsletter to participants.
    4. Maintain command awareness of program by conducting annual summary briefings.
    5. Stock literature stands with handouts and monitor consumption at least every six weeks.
    6. Monitor attendance at TH program activities.

  2. Twenty five percent of participants in the TH program will exhibit positive lifestyle changes six months after participating in a health program intervention, such as the civilian fitness program.

    1. Identify the targeted health risk(s) of participants in an intervention program, and reassess the specific risk six months later.
    2. Offer awareness/education activities and intervention activities annually, based on participant needs assessment and interest survey.

  3. Fifty percent of participants will subjectively affirm having improved morale/attitude or claim to benefit from the program.

    1. Administer a survey to Supervisors of participants to identify changes in morale/attitude
    2. Administer a survey to employees attending TH programs to monitor their attitudes and their perceived benefits/disadvantages of the activity

 

EVALUATION

PROCESS:

  1. Report on record of marketing strategies used to advertise TH program activities. (Goal 1/Obj A)
  2. Report when newsletter printed and mailed to participants. (Goal 1/Obj C)
  3. Evaluate activities of program in annual summary briefing to command. (Goal 1 - 3)
  4. Report usage of literature stands for employee awareness. (Goal 1/Obj E)
  5. Report attendance at TH program activities - total attendance at each activity, total number of different employees attending, etc. (Goal 1/Obj F)
  6. Report activity level of participants in civilian fitness program, and if needed, reasons for lack of activity. (Goal 2/Obj A)
  7. Report results of needs assessment/interest survey and plan for meeting those needs.

OUTCOME:

  1. Compare results of pretest and post-test for TH program activities for improvement in awareness/knowledge. (Goal 1/ Obj B)
  2. Fitness testing of participants in civilian fitness program by pretest and post-test. (Goal 2/Obj A)
  3. Assess effectiveness of civilian fitness program to make lasting behavior changes in exercise by issuing follow-up surveys at one month, three months, and six months post-program. At six months, administer fitness assessment to individuals still exercising consistently. (Goal 2/Obj A)
  4. Assess changes in participants of interventions by comparing behaviors/attitudes before, during, immediately after, three months after, and six months after conclusion of intervention.

  5. Assess effect of intervention program on sick leave days by comparing usage by those in civilian fitness program and those not actively exercising, and by comparing participants' sick leave usage before, during, and after participation in the program. (Goal 2 and 3)
  6. Survey participants in civilian fitness program and their Supervisors to evaluate attitude changes and perceived benefits of intervention. (Goal 3/Obj A,B)
  7. Determine if interventions were able to pick up any undiagnosed problems in participants.
    (Goal 2 and 3/Obj B)
  8. Assess effectiveness of civilian fitness program by survey given to participants requesting information regarding injury. (Goal 3/Obj B)

 

NOTE: The purpose of awareness/education activities is to increase the individual's awareness in a topic to hopefully motivate them from one stage of change to the next. Therefore, the awareness/education activities will not be monitored for changes in behavior. Instead, evaluations will be addressed through outcome #1 above. Intervention programs will be monitored for immediate and long-term behavior changes.
